We begin by reviewing your needs through discussions with involved family members; when necessary we consult with the physician of the care recipient, social workers, hospitals, or nursing home staff.

Next, working in conjunction with you and your family, I will develop an individualized program to manage your daily needs.

After I come into your home, I will continue my unique system of continued personalized contacts through telephone check-in, home visits. Most important, I will continually communicate with you and the family
